Job summary

Qualifications
- Licensed pharmacist in Illinois (IDFPR).
- Bachelor’s degree in Pharmacy required.
- Clinical work experience in a hospital setting.
Responsibilities
- Direct patient care to improve prescribing, administration, and monitoring.
- Provide MTM services and patient education as defined by state act.
- Determine optimal medication doses based on patient factors.
- Collaborate with hematology/oncology and inpatient pharmacy teams.
- Prepare investigational and standard chemotherapy for trials.
- Attend rounds and provide oncology pharmacy services.
