Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington in Texas is seeking a Clinical Navigator to be the face of our hospital, cultivating referral relationships and guiding patients through their rehab stay. You will meet patients at difficult times and highlight the potential success they can achieve with our care.

You will work under the Director of Business Development, coordinating with Admissions and nursing staff to optimize referrals, assess patients, and support service

Qualifications

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.

Responsibilities

• Cultivates referral relationships, manages assigned territory, and completes patient assessments. Makes sales contacts as well as analysis of referral and admission data from assigned territory.
• Works collaboratively with the Admissions Manager, nursing and other internal and external staff to facilitate referral conversion and provides feedback for program development and quality improvement initiatives.

Education

Degree in a health-related field

Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy

Prior marketing and/or rehabilitation/LTACH experience preferred
