Job summary

PAM Health Specialty Hospital of Jacksonville is seeking a Clinical Navigator to be the face of our hospital, meeting patients at difficult times and guiding them through rehab stays with support and care.

Under the Director of Business Development, you will cultivate referral relationships, manage a territory, analyze data, and collaborate with admissions and clinical staff to improve patient experiences and outcomes.

Qualifications

• Education: Degree in a health-related field from an accredited college or university preferred.
• Licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Two years’ experience in the designated field of license; rehabilitation/LTACH and marketing experience preferred.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and makes sales contacts.
• Analyzes referral and admission data from the territory.
• Collaborates with Admissions Manager, nursing and staff to facilitate referrals.
• Provides feedback for program development and quality improvement.
• Builds relationships with patients, families, physicians, nurses, and payers.

Skills

Marketing experience

Two years experience in licensed field

Education
