PAM Health Rehabilitation Hospital of Sugar Land is seeking a Clinical Navigator to cultivate referral relationships, manage a defined territory, and complete patient assessments. The role involves making sales contacts and analyzing referral and admission data in collaboration with the Admissions Manager and nursing staff.

The position emphasizes building strong relationships with physicians, discharge planners, case managers, and payer representatives to optimize referrals and patient care

Qualifikationen

• Must have a health-related degree from an accredited college or university (preferred).
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Minimum of two years’ experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.

Aufgaben

• Cultivate referral relationships and manage assigned territory.
• Complete patient assessments and conduct sales contacts.
• Analyze referral and admission data for the territory.
• Collaborate with Admissions Manager, nursing staff, and internal/external teams.
• Provide feedback and recommendations for program development and quality improvements.
• Maintain excellent relationships with patients, families, physicians, nurses, discharge planners, case managers and payer representatives.
