Job summary

PAM Health Rehabilitation Hospital of Humble in Humble, TX is seeking a Clinical Navigator. You will be the face of our hospital, developing referral relationships, coordinating patient admissions, and supporting patients and families through rehab planning.

This full-time role reports to the Director of Business Development, requires a health-field degree and licensure, and at least two years of relevant experience; we offer comprehensive benefits and a supportive PAMily culture.

Qualifications

• Education: Degree in a health-related field from an accredited college.
• Licensure: Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Experience: At least two years in the designated licensed field; LTAC/marketing experience preferred.

Responsibilities

• Under the Director of Business Development, cultivates referral relationships and manages territory.
• Completes patient assessments and makes sales contacts; analyzes referral/admission data.
• Works with Admissions Manager, nursing staff and payers to facilitate referrals.
• Provides feedback for program development and quality improvement.
• Builds strong relationships with patients, families, physicians, discharge planners and payer representatives.

Education

Degree in a health-related field

Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy

Experience in designated field (minimum 2 years)
