Job summary

PAM Health in Texas is seeking a Clinical Navigator to be the face of our hospital. You will cultivate referral relationships, manage a defined territory, and complete patient assessments to support admissions.

Collaborate with the Director of Business Development, Admissions staff, nurses and payer representatives to enhance referral processes and patient satisfaction. A health-related degree and licensure are preferred and often required.

Qualifications

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.

Responsibilities

• Cultivates referral relationships, manages assigned territory, and completes patient assessments.
• Makes sales contacts as well as analysis of referral and admission data from assigned territory.
• Works collaboratively with admissions staff, nursing and other internal and external staff to facilitate referral conversion and improve patient and payer satisfaction.
