Job summary

PAM Health Specialty and Rehabilitation Hospital of Luling is seeking a Clinical Navigator to cultivate referral relationships, manage a designated territory, and complete patient assessments. You will work with the Admissions team, nursing staff, and payer representatives to drive referrals and improve patient access to rehabilitation services.

Ideal candidates have a health-related degree with current licensure and at least two years in rehabilitation or LTACH settings, plus marketing

Qualifications

• Degree in a health-related field from an accredited college or university is preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech therapy is required.
• Minimum of two years’ experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and conducts sales contacts.
• Analyzes referral and admission data from the territory.
• Collaborates with Admissions Manager, nursing staff, and payer representatives to facilitate referrals.
• Provides feedback for program development and quality improvement relating to customer service and referrals.

Education

Health-related degree from accredited college/university

Current licensure in nursing/therapy (any listed field)

Two years’ experience in the designated field

Marketing and/or rehabilitation/LTACH experience preferred
