Job summary

PAM Health Rehabilitation Hospital of Humble is seeking a Clinical Navigator to cultivate referral relationships, manage a defined territory, and complete patient assessments. You will work with the Admissions Manager, nursing staff, and internal/external teams to optimize referrals and admissions.

This role emphasizes building strong relationships with physicians, hospital staff, and payers. Education in a health-related field is preferred and current licensure in nursing, respiratory,

Qualifications

• Two years’ experience in the designated field of license preferred.
• Rehabilitation/LTACH experience and marketing background preferred.

Responsibilities

• Cultivate referral relationships and manage assigned territory.
• Complete patient assessments and make sales contacts.
• Analyze referral/admission data and coordinate with admissions staff.
• Collaborate with the Admissions Manager and nursing staff to facilitate referrals.
• Provide feedback for program development and customer service improvements.
• Maintain relationships with physicians, discharge planners, case managers, and payer representatives.
