Job summary

Lifepoint Rehabilitation is seeking a Clinical Liaison for Henry Ford Macomb - Oakland Hospital to build referral networks, coordinate intake, and assess admission suitability for the acute rehab unit. This full-time role collaborates with patients, families, payors, and hospital staff to drive appropriate admissions.

Applicants should hold a clinical license and a bachelor’s degree. Prior marketing/sales or EMR experience is preferred, and a valid driver’s license is required.

Qualifications

• Clinical licensure in RN, PT, OT, SLP or RRT and a bachelor’s degree.
• Previous marketing/sales experience preferred.
• Clinical experience with demonstrated clinical assessment skills and EMR systems.
• Formal Sales Training preferred; Valid driver’s license required.

How you'll contribute

• Builds and maintains referral relationships to drive appropriate admissions to the acute rehab unit
• Assist and coordinate the intake and pre‑admission screening process
• Perform on‑site (typically in a referring facility) clinical judgment to determine the appropriateness of the patient for admission
• Communicates rehabilitation and facility options to patients and families.
• Communicates with patients, determines admission appropriateness, and assesses the patient's expectations and requirements for participation.
• Secures information relating to patients' resources and benefits
• Functions as initial contact for external case management and discharge planner personnel with payors and hospital facilities

What we're looking for
