Responsibilities

• Build and maintain strong professional relationships with medical professionals, hospitals, and other healthcare facilities and organizations to support care coordination and referrals.
• Promote the organization’s philosophy, mission, and objectives both internally and externally.
• Attend facility meetings, calls, and in-services to support collaboration and communication across care settings.
• Demonstrate honesty, integrity, and professionalism in all interactions and protect patient information and hospital property.
• Maintain accurate referral-tracking logs to support admissions and coordination.
• Prepare and distribute end-of-day reports to department leaders for clear communication.
• Collaborate with interdisciplinary teams to support transitions of care and address resident needs.
