Qualifications

• Associate degree in a related field is required; a bachelor’s degree is preferred.
• Current RN, LPN, or LSW licensure is preferred.
• Valid driver’s license is required for facility travel.
• Ability to travel between facilities and partner organizations is essential.

Responsibilities

• Build and maintain relationships with medical professionals, hospitals, and healthcare facilities to support care coordination and referrals.
• Promote Marquis Health Services’ philosophy, mission, and care standards internally and externally.
• Attend facility meetings, calls, and trainings to support collaboration and education across care settings.
• Demonstrate professionalism and protect patient information and hospital property.
• Maintain referral-tracking logs to support admissions and coordination.
• Prepare end-of-day reports for department leaders to ensure alignment and communication.
• Collaborate with interdisciplinary teams to ensure smooth transitions of care for residents.
