Qualifications

• Strong communication and relationship-building skills with healthcare partners.
• Requires valid driver''s license for travel between facilities.
• RN/LPN/LSW licensure preferred.
• Associate''s degree required; bachelor''s degree preferred.

Responsibilities

• Build and maintain strong relationships with medical professionals, hospitals, and facilities to support care coordination and referrals.
• Promote the organization''s mission and care standards to internal and external stakeholders.
• Attend facility meetings, calls, and trainings to support collaboration and communication across settings.
• Demonstrate honesty, integrity, and professional handling of patient information and hospital property.
• Maintain referral-tracking logs to support admissions and coordination.
• Prepare and distribute end-of-day reports to department leaders for operational alignment.
• Collaborate with interdisciplinary teams to ensure smooth transitions of care.
