Build and maintain strong, professional relationships with medical professionals, hospitals, and other healthcare facilities and organizations to support effective care coordination and referrals.
Promote the organization’s philosophy, mission, and objectives both internally and externally, serving as a trusted representative of our care standards and values.
Attend and actively participate in facility meetings, calls, and in-services to support collaboration, education, and strong communication across care settings.
Demonstrate honesty, integrity, and professionalism in all interactions, and ensure the respectful handling of patient information and hospital property.
Maintain accurate, up-to-date referral-tracking logs to support efficient admissions and care coordination.
Prepare and distribute timely end-of-day reports to department leaders to ensure clear communication and operational alignment.
Collaborate closely with interdisciplinary teams to support smooth transitions of care and ensure resident needs are clearly communicated and addressed.
Valid driver’s license required, as travel between facilities and partner organizations is essential.
Current RN, LPN, or LSW licensure preferred.
Associate’s degree in a related field required; Bachelor’s degree preferred.
Strong communication and relationship-building skills with the ability to engage effectively with healthcare professionals and community partners.
Ability to work collaboratively with multiple residents, patients, and care teams in a fast-paced healthcare environment.
Strong organizational skills with attention to detail and accuracy in documentation and reporting.
