Crossroads Inc in Chandler, AZ is seeking a Clinical Lead to support the Clinical Coordinator in organizing location-specific clinical operations, overseeing client services, treatment planning, and discharge planning. The role involves aiding staff, coordinating care, and managing crises as needed.

The position requires handling client caseloads, ensuring timely documentation, and supporting staff with complex cases. Travel between sites may occur; a valid AZ driver's license is preferred.

Qualifications

• Strong written and verbal communication skills.
• Proficient in Computer/Microsoft Office/EMR and typing.
• High ethical standards and sound judgment.

Responsibilities

• Maintain client caseload and complete documentation (progress notes, evaluations, treatment plans, discharge planning).
• Assist Case Managers and Peer Mentors; organize coordination of care efforts.
• Complete client intake and discharge processes with comprehensive assessments and treatment plans.
• Support training of clinical team and provide individual/group interventions including crisis management.
• Coordinate care through case reviews and weekly progress updates; assist in discharge planning.
