Job summary

This role involves maintaining caseloads, documenting progress, and coordinating care across teams.

The position requires at least a high school diploma or GED and 1-2 years of related experience, CPR/First Aid/Narcan training, and valid clearance.

Qualifications

• Knowledge of addiction and recovery concepts.

Responsibilities

• Develops and documents individual treatment plans and tracks progress.
• Manages a client caseload and completes required documentation on time.
• Provides individual and group interventions including crisis management.
• Coordinates care and discharge planning with treatment teams.
• Participates in clinical meetings, trainings, and program activities.
• Assists clients with applications for government assistance and referrals.
• Ensures compliance with policies, licensure, and regulatory rules.
