Job summary

Crossroads is seeking a Clinical Coordinator to provide clinical intervention services to adults in residential and outpatient care, under the Residential Clinical Program Manager. You will conduct screenings, assessments, treatment planning, case load management and crisis coordination with limited supervision.

Responsibilities

• Oversees all aspects of the clinical team and scheduling.
• Conducts assessments or screenings to determine patient needs.
• Develops and monitors individual treatment plans and progress.
• Prepares intake documentation, progress notes, and other client records.
• Monitors staff caseloads and ensures timely documentation.
• Provides individual and group interventions including crisis management.
