Job summary

Oak Street Health is seeking a full-time Medical Scribe in Memphis to support primary care providers with real-time documentation. You will observe patient visits, document encounters, and assign CPT/ICD-10 codes while learning our value-based care workflows.

The role emphasizes accuracy in clinical documentation, population health data, and collaboration with the care team. Training is provided; commitment of 1–2 years is encouraged and hours are typically 40 per week.

Qualifications

• Knowledge of medical terminology and common medications (required).
• Prior clinical experience, including shadowing and/or volunteering (preferred).
• Prior scribe or transcription experience (preferred but not required).

Responsibilities

• Documenting Patient Encounters 80%: observe visits and document in a structured note with HPI, assessment, plan, and exam.
• Clinical Documentation Improvement 10%: review medical records and support documentation improvements.
• Administrative support for provider and care team 10%: placing orders, referrals, and related tasks.
• Other duties as assigned.
