Laurel Ridge Treatment Center is seeking a Post-Acute Services Director to drive growth by cultivating hospital relationships and managing patient referrals. You will oversee transitions, discharge processes, and sales/clinical programming strategies to expand market presence.

Role focuses on converting referrals, building brand loyalty, and meeting admissions targets across nursing centers with data-driven strategies.

Qualifications

• RN or LPN licensure preferred, but not required, except where mandated in the state in which employment occurs.
• Valid driver’s license.
• Five to seven years of recent clinical experience.

Responsibilities

• Lead the patient referral process from review through to acceptance.
• Establish relationships with hospital referral sources and key decision-makers.
• Communicate the nursing centers’ clinical strengths to drive referrals.
• Demonstrate 60%+ conversion for clinically and financially appropriate referrals.
• Develop market strategy plans to grow admissions by at least 5% per period.
