Job summary

Genesiscareers seeks a Post-Acute Services Director to drive growth for assigned nursing centers by managing patient referrals and transitions. You will build and maintain relationships with hospital systems, and oversee the implementation of sales and clinical programming strategies to expand market presence.

Qualifications

• RN or LPN licensure preferred, but not required, except where mandated by state law.
• Valid driver’s license.
• Five to seven years of recent clinical experience.

Responsibilities

• Lead patient referral process from review to admission, ensuring smooth clinical handoffs.
• Establish and nurture hospital referral source relationships, developing 5–10 new contacts monthly.
• Communicate the nursing centers’ clinical strengths to hospitals to drive referrals.
• Achieve a conversion rate of 60% or higher for clinically and financially appropriate referrals.
• Develop a market strategy plan with data analytics to meet admissions and sales targets (at least +5% per period).
