Genesis Rehab Services is seeking a Post-Acute Services Director to drive growth across assigned nursing centers by cultivating hospital referrals, overseeing patient transitions, and ensuring smooth clinical handoffs. The role emphasizes building durable referral networks, communicating center strengths, and implementing strategic programming to sustain market presence.

The ideal candidate brings 5–7 years of clinical experience, with RN/LPN licensure preferred and a valid drivers license.

Qualifications

• RN or LPN licensure preferred, but not required, except where mandated by state.
• Valid drivers license.
• Five to seven years of recent clinical experience.

Responsibilities

• Lead the patient referral process from review through to acceptance, ensuring smooth admissions.
• Utilize the CRM system to document handoffs and improve patient outcomes and financial performance.
• Develop 5–10 new hospital contacts per month; build loyalty for the centers clinical offering.
• Communicate clinical strengths of centers to align with hospital referrals and drive flow.
• Aim for a conversion rate of 60%+ for appropriate referrals; monitor trends 90–120 days post-hire.
• Develop market strategy plans and meet/exceed admissions goals with at least 5% growth per period.
