Job summary

PowerBack Rehabilitation is seeking a Post-Acute Services Director to drive growth at our nursing centers through hospital referrals and strong leadership. You will oversee patient transitions, manage CRM-based workflows, and build durable relationships with hospital systems to sustain market presence.

Ideal candidates bring RN or LPN licensure, 5–7 years of clinical experience, and a proven ability to convert referrals.

Qualifications

• RN or LPN licensure preferred.
• Valid driver’s license.
• Five to seven years of recent clinical experience.

Responsibilities

• Lead the patient referral process from review through admission, ensuring smooth clinical handoffs.
• Cultivate hospital referrals and 5–10 new contacts per month within hospital systems.
• Articulate nursing center clinical strengths to align with hospital needs and drive referrals.
• Achieve 60%+ conversion for appropriate referrals and improve trends within 90–120 days.
• Develop market strategy plans with data analytics to exceed admissions and sales goals by at least 5% per period.
