Job summary

Enhabit Home Health & Hospice seeks a proactive business development professional to manage relationships with referral sources, educating partners on services and program requirements to improve post-acute care transitions. Qualifications

• Bachelor's Degree in healthcare administration, marketing, or related field or graduate of an approved school of nursing, therapy or social work.
• Graduate of an approved nursing, therapy or social work program is highly preferred.
• Minimum two years of experience in healthcare business development, community relations, marketing, or sales.
• Familiar with federal, state, and local laws governing Medicare home health and hospice operations.
• Must possess a valid state driver’s license and automobile liability insurance.
• Strong interpersonal, communication and presentation skills.
• Clinical license required where applicable.
• Experience with home health, hospice, or post-acute care is preferred.

Responsibilities

• Serves as the primary relationship manager with referral sources including physicians, hospitals, health systems, and senior living communities to grow referrals.
• Educates referral sources and patients about Enhabit services and program requirements to aid transitions.
• Supports post-acute care transitions to improve patient outcomes and satisfaction.
• Builds and maintains partnerships with physicians, hospitals, and community organizations to advance care in the home.
