Job summary

Enhabit Home Health & Hospice is seeking a proactive professional to act as the primary liaison with referral sources, including physicians, hospitals, and health systems. You will educate on services and program requirements to support post-acute care transitions and improve patient outcomes.

Qualifications

• Bachelor's degree in healthcare administration, marketing, or related field; or graduate of nursing, therapy, or social work programs.
• Minimum two years of healthcare business development, community relations, marketing, or sales.
• Knowledge of federal, state, and local laws governing Medicare certified home health and hospice operations.

Responsibilities

• Serve as the primary relationship manager between Enhabit and referral sources including physicians, hospitals, and health systems.
• Educate referral sources and patients about Enhabit services and program requirements to improve post-acute transitions.
• Drive referral growth and ensure high patient/referral source satisfaction through effective communication and collaboration.
