Formación

• Bachelor's degree in healthcare administration, marketing, or related field, or nursing/therapy/social work degree.
• Minimum two years in healthcare business development, community relations, marketing, or sales.
• Knowledge of federal, state, and local laws governing Medicare home health and hospice operations.
• Valid driver’s license and automobile liability insurance.

Responsabilidades

• Serve as the primary relationship manager between the company and referral sources (physicians, hospitals, health systems, senior living communities, associations).
• Support referral growth by educating sources on services and program requirements to navigate post-acute care transitions.
• Improve patient outcomes and satisfaction through effective referral management and care transitions.
