Job summary

CVS Health Corporation is hiring for a full-time clinical role in Louisiana. The incumbent will apply advanced clinical judgement to plan member care, coordinate psychosocial services, and work with providers to optimize outcomes.

Travel up to 125 miles for one-day field visits and 2 days weekly until 9pm are required. The position requires a Master’s Degree and valid licensure (LCSW/LPC/LMFT), strong communication skills, and a focus on documentation and regulatory compliance.

Qualifications

• Current/Active Clinical Licensure (LCSW, LPC, LMFT).
• Case Management experience.
• Strong customer service orientation and problem solving.
• Excellent Communication/Telephonic skills.
• Excellent motivational interviewing skills and ability to build rapport telephonically.
• Excellent documentation and compliance with regulatory standards.

Responsibilities

• Apply advanced clinical judgement to assess and plan member care.
• Direct provider collaboration and coordinate psychosocial wraparound services.
• Instruct the application of available resources to produce optimal, cost-effective outcomes.
