Job summary

CVS Health in Louisiana is seeking a Clinical Case Manager Behavioral Health to provide advanced assessment, care planning, and coordination of psychosocial services for members in need of integrated health support.

This full-time, telework role requires an active Louisiana independent behavioral health license, 3+ years post-master’s clinical experience, up to 50% in-state travel, and a master's degree. Competitive benefits and a pay range of $54,095–$116,760 with bonus eligibility.

Qualifications

• Must reside in Louisiana.
• In-state travel up to 50% of the time.
• Unencumbered Independent Behavioral Health Clinical license in the state of Louisiana (e.g., LCSW, LPC, LMFT).
• 3+ years of direct clinical practice experience post master's degree, with general youth or pediatrics population in a hospital setting, alternative care setting such as ambulatory care, residential, or outpatient clinic/facility.
• 2+ years of experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint, etc.).

Responsibilities

• Assess member needs/eligibility and develop plans of care.
• Coordinate with providers and wraparound services for cost-effective outcomes.
• Conduct crisis intervention and follow-up for behavioral/medical crises.
• Collaborate with Medical Directors and other programs; present at case conferences.
• Monitor and document care in accordance with regulatory and accreditation guidelines.
