Fort Defiance Indian Hospital Board, Inc. is seeking a qualified healthcare professional to develop and implement comprehensive care plans for patients, coordinating with families and a multi-disciplinary team to update plans as patient needs evolve.

The role emphasizes chronic care management, education, crisis referrals, and collaboration with community and tribal services to improve patient outcomes and access to care in the Navajo Nation service area.

Competenze

• Two years of nursing assistant, medical assistant, or health care related experience in clinical, medical/surgical, critical care, or outpatient setting.
• Associate of Science degree in Nursing, Social Work, Human Services or related field from an accredited program.
• Valid American Heart Association certification in Basic Life Support (BLS).

Mansioni

• Develops and implements comprehensive care plans with patient, family, team and evaluates/updates plans based on needs and assets.
• Coordinates inter-organizationally with family, clinics, inpatient services, mental health and involved services and agencies.
• Educates, counsels, supports and provides patient appropriate anticipatory guidance; intervenes in crises or facilitates referrals.
• Maintains chronic disease registry and uses it to plan and monitor care; applies inclusion/exclusion criteria for patient identification.
• Serves as a contact and resource for family and community partners, including CMS and payer interfaces.
