Fort Defiance Indian Hospital Board, Inc. seeks a licensed pharmacist to provide comprehensive pharmaceutical care across clinics and community sites, including patient counseling, medication history, and education on therapy and potential side effects. The role involves evaluating orders, ensuring safety and regulatory compliance, and collaborating with physicians, nurses, and staff to optimize patient outcomes.

An active U.S. license is required; Navajo Nation preference applies.

Competenze

• Completion of an accredited college or university with a Bachelor's degree in Pharmacy or Pharm.D.
• Active, unrestricted Pharmacy License in any U.S. State or Territory by completing necessary Board of Pharmacy requirement for the respective state of licensing.
• No additional experience required outside of a completed training program as defined in the education qualifications listed below.

Mansioni

• Monitor, modify, and evaluate drug therapy; educate and counsel patients about current drug therapy and side effects.
• Perform pharmaceutical compounding for sterile and non-sterile preparations; screen prescriptions for proper interpretation and dosing.
• Evaluate medication orders for appropriateness based on guidelines and patient factors; make recommendations.
• Dispense medications with attention to accuracy and safety; monitor outcomes and toxicity.
• Participate in medication reconciliation, rounds, and interdisciplinary planning; advise on dosing and monitoring.
