Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure accessible documentation, test results, diagnoses and information for providers.
• Maintain up-to-date patient records including demographics and appointment history.
• Maintain confidentiality and privacy of patient information per regulations.
• Reconcile medications.
• Coordinate patient appointments with providers and care team.
• Interact with patients via email, portal, phone, text, or in person for scheduling and instructions.
• Document and log care management activities accurately.
