Your Health in Greenville, SC is seeking a Clinical Coordinator to support patient care across our clinics. The role involves chart preparation, scheduling visits, coordinating chronic care management activities, and providing provider and patient support.

This full-time, salary-based position operates 8:00 AM–5:00 PM, Monday through Friday, with requirements including CMA/LPN/RN licensure, CPR, and at least one year in a healthcare setting.

Qualifications

• Must be a CMA or licensed nurse with active SC licensure in good standing.
• CPR certification required.
• High school diploma or equivalent required.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Utilize Navina to ensure documents are added, completed, and updated.
• Maintain confidentiality and verify active consent forms and paperwork.
• Reconcile medications.
• Coordinate patient appointments with providers and care team.
• Interact with patients via email, portal, phone, text, in-person to gather information and provide instructions.
• Document and log care management activities.
