Your Health Organization in Greenville is actively seeking a Clinical Coordinator to support clinical needs for our Greenville patients. The role focuses on chart prep, scheduling visits, and logging care management activities to ensure seamless patient care.

The position operates at our Greenville office daily, is full-time and salary-based, with standard 8:00 AM–5:00 PM, Monday–Friday hours. Prior experience in a similar role is valuable in our value-based care model.

Qualifications

• Must have CMA or licensed nurse status and hold a license in the state of employment.
• Minimum of one year healthcare experience preferred.
• Must hold and maintain current CPR certificate.
• High school diploma or equivalent required.
• Strong interpersonal and communication skills under all conditions.
• Ability to foster a cooperative work environment and be a team player.
• Ability to manage multiple responsibilities and exercise sound judgment.
• Ability to read and communicate effectively, with strong written and verbal skills.
• Basic computer knowledge required.
• Must be able to travel between offices and work flexible hours.
• Must be a licensed driver with a valid vehicle.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Utilize Navina to ensure documentation is added, completed, and updated.
• Ensure confidentiality and privacy regulations are followed; verify consent forms in the chart.
• Reconcile medications.
• Schedule and coordinate patient appointments with providers and care team.
• Interact with patients via email, portal, phone, text, or in person to collect information and provide pre-appointment instructions.
• Use communication tools such as phone, email, AthenaText, and Microsoft Teams.
• Document and log care management activities.

We are seeking a Clinical Coordinator to assist with all clinical needs for our Greenville patients. The Clinical Coordinator plays a crucial role ensuing the seamless coordinator of patient care. This position involves proficient chart preparation, communication, scheduling of patient visits, appropriate utilization and logging of chronic care management activities, and provider and patient support.

This position will be working in the Greenville office daily. This is a full-time, salary-based8-hr position (8:00AM-5:00PM) (Monday-Friday).

What Are We Looking For?

Your Health is currently looking for a Clinical Coordinator to join our growing primary care family. A successful Clinical Coordinator will be able to perform these essential duties and responsibilities accurately and efficiently. Prior experience in a similar role is crucial to the success of our team due to the fast past and value-based model of care.

The following is a list of essential functions, which may be subject to change at any time and without
notice. Management may assign new duties, reassign existing duties, and/or eliminate function(s).

Areas of Responsibility:

• Review and organize patient medical records in preparation for visits.
• Ensure all necessary documentation, test results, documents, diagnoses, and relevant information are accessible for the provider. Utilize Navina to ensure these are added, completed, and/or updated.
• Ensure confidentiality and adhere to privacy regulations when handling sensitive patient information. Verify active consent forms and alignment paperwork are completed and in the patient’s chart.
• Reconcile medications.
• Collaborate with care team to schedule and coordinator patient appointments with the Company’s provider(s), and other members of the care team
• Interact with patients via email, patient portal, telephone, text, in-person, etc. to gather necessary information, schedule and confirm appointments in advance, and provide pre-appointment instructions.
• Communication methods such as phone, email, AthenaText, Microsoft Teams, etc. will be utilized.
• Appropriately and accurately document and log care management activities.

Qualifications

• Must be a Certified Medical Assistant (CMA) or licensed nurse (i.e. LPN or RN). Must be licensed in the state of employment. Certification/license must be in good standing with the appropriate board.
• A minimum of one (1) year experience in a healthcare setting preferred.
• Must hold and maintain current CPR certificate.
• High school diploma or equivalent required.
• Should demonstrate strong interpersonal and communication skills under all conditions and circumstances.
• Ability to foster a cooperative work environment.
• Team player with ability to manage multiple responsibilities and demonstrate sound judgment.
• Ability to read and communicate effectively.
• Strong written and verbal communication skills.
• Basic computer knowledge.
• Must be able to work flexible hours and travel between offices, facilities, etc.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working condition.
