Job summary

Oak Street Health in Philadelphia is seeking a Registered Nurse to coordinate patient care, build strong relationships with patients and their support teams, and deliver timely, compassionate nursing in a clinic setting. You will work with an interdisciplinary team, perform triage, point-of-care testing as needed, manage medications, administer injections and vaccines, and use EMR systems to document care.

Anticipated 40 hours per week with a comprehensive benefits package; a $5,000 sign-on

Qualifications

• Active Registered Nurse licensure in good standing (PA).
• Active CPR/BLS certification.
• EMR experience and clinic-based patient care.
• HIPAA compliance and patient confidentiality.
• Strong communication and assessment skills.
• Ability to work independently and manage multiple tasks.

Responsibilities

• Provide competent nursing care in a safe, consistent manner.
• Respond to incoming requests and ensure reliable patient support.
• Perform triage and symptom management whether in person or remotely.
• Follow standardized protocols for medication management and vaccinations.
• Review and update patient medications; educate patients and families.
• Collaborate with care teams to implement and monitor care plans.
• Delegate tasks to clinical team members as appropriate.
• Create an engaging environment focused on equitable care.
