Warren Clinic, Inc. seeks an experienced Practice Improvement Coordinator to partner with clinics and leadership in advancing population health and value-based care initiatives. You will coordinate cross-functional teams, analyze performance data, and drive operational improvements to enhance clinic engagement and outcomes.

The role requires collaboration with providers and administrative leaders, and involves coordinating improvement projects across multiple clinics in a fast-paced setting.

Qualifications

• Minimum 3 years of Care Coordination, Healthcare Operations, Population Health, Practice Management, Quality Improvement, or Value-Based Care experience.

Responsibilities

• Serve as the primary population health and value-based care liaison for assigned clinics.
• Partner with clinics and leadership to identify opportunities for operational improvement and quality performance.
• Analyze clinic performance data and translate into actionable insights to support improvement strategies.
• Coordinate cross-functional support from Care Coordination, Pharmacy, Quality, Risk Adjustment, and other SMEs.
• Support implementation and standardization of clinic engagement processes, workflows, and best practices across clinics.
• Monitor progress toward population health goals and communicate updates and opportunities to leadership.
