Warren Clinic, Inc. is seeking a Practice Improvement Coordinator to serve as the primary clinic-facing partner for population health and value-based care initiatives. You will coordinate across clinics, engage leadership, and translate performance data into actionable improvement plans.

The role requires collaboration with care teams, strong project coordination, and the ability to manage multiple priorities in a fast-paced environment. Occasional travel may be required.

Qualifications

• Minimum 3 years of Care Coordination, Healthcare Operations, Population Health, Practice Management, Quality Improvement, or Value-Based Care experience.

Responsibilities

• Serves as the primary population health and value-based care liaison for assigned clinics.
• Partners with clinics and organizational leadership to identify opportunities for operational improvement and quality performance.
• Analyzes and translates clinic performance data into actionable insights and supports improvement strategies.
• Coordinates cross-functional support to ensure aligned and efficient clinic support.
• Monitors progress toward population health goals and communicates updates to leadership.
