Freeman Health System is seeking a Quality Coordinator - Clinics to support quality improvement initiatives, ensure compliance with patient-focused care models, and drive performance improvements through analytics and staff training. The role collaborates with the Regional Quality Director to close gaps in care and ensure accurate reporting to payers.

You will monitor KPIs, coordinate clinic workflows, and provide ongoing staff education to sustain high-quality care and regulatory adherence

Formación

• Associate degree required in healthcare field.
• Bachelor's degree in nursing preferred.
• 2–4 years in quality improvement or population health in healthcare.

Responsabilidades

• Implements and monitors quality initiatives to meet best practices and regulatory requirements.
• Supports clinic teams as SME on quality workflows and procedures.
• Coordinates patient outreach to close care gaps with timely follow-up.
• Optimizes schedules to address preventive and chronic disease gaps.
• Monitors KPIs and provides feedback to stakeholders.
• Conducts rounding with staff to identify workflow improvements.
• Assists in medical record audits and payer documentation.
• Delivers training sessions and ongoing staff support on quality initiatives.
• Collaborates with data analytics and population health to ensure accurate reporting.
• Maintains compliance with payer quality programs and incentives.
