We are seeking a detail-oriented and organized Health Information Technician to support the accurate, timely, and confidential management of patient health information. The Health Information Technician is responsible for reviewing, compiling, maintaining, filing, and retrieving medical records and related documentation while ensuring records are complete and meet applicable regulatory and organizational requirements.
This position works closely with clinical and administrative departments to maintain accurate health information records and provide authorized information in accordance with confidentiality and privacy requirements.
Key Responsibilities
- Review medical records, reports, and charts for completeness, accuracy, required signatures, and missing documentation.
- Catalog, organize, and prepare medical reports for placement in patient health information files.
- Prepare charts and health information forms for new admissions and other patient-care activities.
- Complete forms and prepare requests for specific medical reports, certificates, and other health information.
- Compile and type statistical reports, including daily and monthly census reports, admissions, discharges, Medicaid days, length of stay, and other required statistics.
- File medical reports and documentation into health information records in accordance with established procedures.
- Maintain logs, files, and tracking systems for health information records.
- Retrieve health information records as requested by authorized personnel.
- Review requests for patient information and provide records or information when the request is appropriate and authorized.
- Coordinate with other departments regarding health information record procedures, documentation requirements, and record availability.
- Maintain patient confidentiality and safeguard protected health information at all times.
- Identify missing or incomplete documentation and follow established procedures to obtain required information.
- Proofread medical reports and documentation and recognize potential errors or inconsistencies.
- Perform other health information and medical records duties as assigned.
Qualifications and Skills
- Knowledge of health information technology and medical record-keeping practices.
- Knowledge of applicable accreditation and regulatory requirements, including JCAH/JCAHO and Medicare/Medicaid medical record requirements, as applicable to the organization.
- Knowledge of patient confidentiality and requirements governing the privacy and release of health information.
- Working knowledge of medical terminology.
- Proficiency with computers, word-processing software, electronic health information systems, and basic office equipment.
- Ability to use calculators and perform basic calculations accurately.
- Strong attention to detail and accuracy when reviewing medical records and reports.
- Ability to identify missing information, signatures, or required documentation.
- Ability to proofread medical reports and recognize errors.
- Ability to organize, gather, compile, classify, and maintain information accurately.
- Ability to work independently while following established policies and procedures.
- Ability to communicate effectively and coordinate with other departments.
