Responsibilities

• Provide brief counseling and recovery-support interventions at least monthly.
• Assess client needs and coordinate referrals across treatment, HIV/HCV, housing, employment.
• Conduct clinic-based check-ins and coordinate with teams to address barriers.
• Maintain confidential case-management records and required data reports.
• Participate in training, supervision and team meetings.
