The Meharry Addiction Clinic is seeking a Clinic-Based Case Manager to provide intensive case management, brief counseling, and care coordination for adults. The role supports linkage to treatment, HIV/HCV services, housing, employment, and other supports to promote ongoing engagement in care.

Primary duties include monthly counseling, thorough assessment and referral coordination, and detailed documentation across systems. Strong collaboration with an interdisciplinary team is essential.

Qualifications

• Knowledge of substance use disorders, harm reduction, recovery services, and HIV/HCV resources.
• Ability to assess client needs, develop service plans, and coordinate referrals.
• Ability to recognize crises and respond calmly.
• Strong written, verbal, interpersonal, and documentation skills.
• Maintain confidentiality and professional boundaries in diverse settings.
• Organize caseload, prioritize tasks, and work with an interdisciplinary team.
• Proficiency with Word, Excel, PowerPoint, EHRs, and data systems.

Responsabilités

• Provide brief counseling and recovery-support interventions at least monthly.
• Assess client needs and coordinate linkage for treatment, housing, employment, and other services.
• Conduct clinic-based check-ins and coordinate with staff and partners for continued engagement.
• Maintain confidential case-management records and logs in required systems.
• Compile and submit program activity data and reports to department and funders.
• Stay current on treatment options, patient-assistance programs, and community resources; participate in trainings.
