Job summary

Meharry Medical College seeks a Medical Case Manager to coordinate comprehensive HIV care under the supervision of a Clinical Psychologist. The role acts as a clinic liaison with community agencies and medical providers to keep patients engaged in care.

You will manage adherence counseling, care coordination, and referrals, while enrolling patients in Ryan White and supporting transitions for those hospitalized or incarcerated.

Qualifications

• Master's degree in a health-related field is preferred.
• Bachelor's degree with a minimum of two (2) years of directly related work experience will be considered after special evaluation.
• Familiarity with patients living with HIV is highly desirable.

Responsibilities

• Coordinate a comprehensive patient care plan under supervision of the Clinical Psychologist.
• Serve as clinic liaison between patients, medical providers, and community agencies to maintain contact and ensure HIV care.
• Maintain a caseload to ensure all patients receive comprehensive care.
• Provide adherence counseling and care coordination, including linkage, reengagement and referrals.
• Enter data in CAREWare templates and submit required reports.
• Attend required meetings and monitor patient transitions and outcomes.
• Enroll and certify eligible patients into Ryan White program and ensure requirements are met.
