Job summary

Mental Health Cooperative, based in Nashville, seeks a Substance Use Services Intake Clinician to conduct comprehensive assessments and determine care pathways. The role requires onsite work with potential remote options based on need and supervision.

You will coordinate enrollments, referrals, and documentation, collaborating with psychiatrists, nurses, therapists, and care managers to support enrollment and treatment planning.

Qualifications

• Master's degree in Social Work, Counseling, or related behavioral health field.
• Licensed or license-eligible in Tennessee (LCSW, LMSW, LPC/MHSP, LMFT).
• Valid Tennessee Driver’s License with acceptable MVR and background.
• Ability to work onsite and meet productivity targets.

Responsibilities

• Conduct comprehensive substance use and behavioral health intake assessments.
• Determine appropriate level of care and make treatment recommendations.
• Coordinate and submit ROIs to providers and obtain records.
• Collaborate with hospitals, primary care, and community partners.
