Job summary

Qualifications

• Master’s degree required; LADAC II may be considered
• Minimum 2 years’ experience in counseling or substance use treatment
• Licensure or license eligibility in social work or counseling

Responsibilities

• Provide individual counseling using evidence-based modalities
• Complete assessments, treatment plans, progress notes, and discharge documentation
• Facilitate group counseling, psychoeducation, and skills-based interventions
• Provide case management by coordinating referrals and linking to resources
• Enter and maintain accurate patient and service information in TN-WITS and EHR
• Build collaborative relationships with clinic staff and outside providers, attend meetings and trainings
