Responsibilities

• Guides clients through the HHS registration process at orientation.
• Evaluates eligibility for tribal and community services and funding.
• Assists with applying for and acquiring benefits as a Tribal Assister.
• Verifies enrollment and insurance details and documents in the EHR.
• Coordinates care with providers and external agencies as requested.
• Documents client contacts and care activities within required timelines.
