Job summary

SafeinHome is seeking an Entry-Level Case Manager to support onboarding, planning, and ongoing client services. You will work with a cross-functional team and interact with clients, families, and support circles to develop personalized plans that reflect goals and abilities.

In this role, you will manage onboarding processes, maintain confidential materials in a compliant manner, and communicate via phone and email.

Qualifications

• Ability to communicate effectively via phone and email.
• Maintain confidentiality of PHI and PII.
• Read and create the Person-Centric Individualized Service Plan (PCISP) and related Support Plan.
• Independently manage onboarding pipeline and client base.

Responsibilities

• Collaborate with clients, families, and support teams to develop individualized plans.
• Monitor and adjust plans to align with client goals.
• Maintain accurate case notes, documentation, and reports.
• Coordinate with providers and stakeholders to ensure continuity of support.
• Attend team meetings, trainings, and ensure regulatory compliance.
