Job summary

ALLCARE HOMECARE LLC in Long Beach, CA is seeking a compassionate Case Coordinator to join our home care team. The ideal candidate will travel to client homes and work directly with clients, families, and care providers to assess needs and build individualized care plans.

Responsibilities include coordinating services and appointments, monitoring progress, maintaining documentation, and providing emergency direct care coverage when needed. Regular in-person visits are required.

Qualifications

• Healthcare experience in home care or case management preferred.
• Experience with individuals with disabilities is a plus.
• Current CPR/First Aid certification is required.
• Strong communication, organization, and problem-solving skills.
• Reliable transportation and willingness to travel to client homes.
• Ability to work independently and manage multiple client needs.
