Job summary

ALLCARE HOMECARE LLC is seeking a compassionate Case Coordinator to join our home care team. You will assess client needs, develop care plans, coordinate services, monitor progress, and ensure compliance with HIPAA and agency policies.

Responsibilities include conducting assessments, coordinating services with clients and providers, visiting clients, maintaining documentation, and providing emergency coverage when needed.

Qualifications

• At least 1 year of healthcare experience (home care or case management preferred).
• Experience working with individuals with disabilities is a plus.
• Current CPR/First Aid certification.
• Strong communication, organization, and problem-solving skills.
