Job summary

Senior Helpers of Baltimore & Central Maryland is seeking a Client Care Coordinator to support client care delivery from intake through ongoing service coordination. You will coordinate client intake, non-clinical assessments, service plans, and timely onboarding communications with clients and families.

You will report to the Director of Operations and collaborate with nursing staff, caregivers, and the business development team to ensure continuity of care, high-quality service, and compliance

Qualifications

• Bachelor’s degree preferred in healthcare administration, social work, gerontology, human services, business, nursing, or related field.
• 1–3 years of experience in home care, healthcare, client services, senior services, customer service, or care coordination preferred.
• Experience working with seniors, families, or healthcare clients strongly preferred.
• Strong communication, organization, customer service, and relationship-building skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Proficiency with Microsoft Office and healthcare software platforms such as WellSky preferred.
• Valid driver’s license and reliable transportation required.
• Ability to travel locally for client visits, assessments, and supervisory visits.

Responsibilities

• Coordinate client intake visits and non-clinical assessments.
• Develop, coordinate, and maintain client service plans under leadership and clinical staff.
• Maintain accurate client census tracking and communicate updates to leadership.
• Communicate with clients, families, caregivers, scheduling staff, and operations.
• Support continuity of care and timely service transitions across teams.
• Ensure compliance with agency policies and Maryland regulations.
