Job summary

Susquehanna County Interfaith is seeking a compassionate Case Manager to deliver client-centered support within our programs. You will conduct intake, assessment, goal setting, and follow-up while coordinating with treatment providers and community partners.

The role involves assisting individuals and families to identify barriers, access resources, and build skills for greater stability and independence, including potential group support and program facilitation as assigned.

Qualifications

• Associate’s, undergraduate, or graduate degree in social work or related field.
• Experience in social services, nonprofit organizations, or case management.
• Drug and Alcohol certification or willingness to obtain certification within established timeframe.
• Excellent interpersonal and customer service skills.
• Strong communication, organization, and documentation skills.
• Ability to build trusting relationships while maintaining appropriate professional boundaries.
• Ability to work independently while collaborating with staff and community partners.
• Strong problem-solving, critical-thinking, and decision-making skills.
• Compassion, patience, and discretion with individuals facing challenges.
• Commitment to Interfaith’s mission, core values, and beliefs.

Responsibilities

• Provide client intake, assessment, support, and follow-up for Interfaith programs as approved by the Executive Director and Board of Directors.
• Develop individualized goals and action plans with clients based on needs, strengths, and barriers.
• Provide ongoing case management, advocacy, and accountability to help clients progress toward goals.
• Educate clients about available community resources and assist access to supports.
• Maintain regular communication and follow-up with clients to monitor progress.
• Maintain accurate, timely, and confidential client records and documentation.
• Participate in Pathways Program, grief support groups, SMART Recovery, and other programs as assigned.
• Coordinate services and referrals with treatment providers and community partners.
