Job summary

Right at Home West Palm Beach & Vero Beach is seeking a Care Manager to serve as the trusted liaison among clients, families, and caregivers. The role focuses on assessing needs, building relationships, and guiding care coordination to ensure clients receive consistent, high-quality service.

The candidate will lead client assessments, orient new caregivers, review performance reports, and collaborate with HR to support staff and operations, while maintaining accountability and compassionate care

Qualifications

• Experience in home care, senior services, care coordination, case management, nursing or social services.
• Strong communication, listening, and empathy.

Responsibilities

• Serve as a primary liaison between clients, families, caregivers, and the Right at Home team, ensuring concerns and information are addressed.
• Conduct initial client assessments to understand needs, goals, and home environments.
• Build strong client relationships through regular communication, follow-up, and responsiveness.
• Conduct new-hire orientations and explain expectations, culture, policies, and quality care commitments.
• Regularly review RightAssist reports, identify trends and follow up items with leadership.
• Manage and support the Guide Program to provide guidance and resources to clients and caregivers.
• Assist with HR activities, including onboarding, documentation and employee communication.
• Support caregiver engagement and accountability, maintaining high standards of care.
• Identify and communicate changes in client needs, caregiver performance, scheduling, or service issues.
• Participate in client and caregiver follow-up to resolve issues before they escalate.
