MyCare Medical in Palm Beach Gardens, FL is seeking a full-time Registered Nurse Care Manager to join our physician-founded primary care team. The RN CM will perform care coordination and case management in patients' homes and community settings, focusing on complex medical and social needs.

Formación

• Active, unrestricted RN license in Florida.
• Minimum 2 years of nursing experience.
• At least 1 year in case management, home health, community health, public health, or care coordination preferred.
• Experience with Medicare Advantage, seniors, chronically ill, or medically complex populations preferred.
• Knowledge of Social Determinants of Health (SDOH) and community resources.
• Strong care coordination, communication, organization, and critical-thinking skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Travel regularly for in-home patient visits.

• Conduct comprehensive assessments in patients' homes and community settings.
• Identify barriers to care including housing, transportation, and social support.
• Develop and update individualized patient-centered care plans.
• Coordinate care among primary care providers, specialists, social workers, and home health agencies.
• Connect patients with community resources and social services.
• Support transitions of care and follow-up to reduce hospitalizations.
• Educate patients and caregivers on disease management and self-management.
• Document assessments, care plans, interventions, and outcomes.
• Promote patient engagement and adherence.
