Job summary

Right at Home West Palm Beach & Vero Beach is hiring a Care Manager who acts as a trusted liaison among clients, families, and caregivers to ensure clear communication and timely problem-solving. You will conduct initial client assessments and help families understand how Right at Home can support them.

You’ll stay connected as care evolves, monitor reports, and help ensure clients receive consistent, high-quality care with accountability and empathy.

Qualifications

• Experience in home care, senior services, care coordination, case management, nursing or social services is highly valued.
• Strong communication and relationship-building skills.
• Organized, proactive, and accountable with the ability to manage multiple priorities.

Responsibilities

• Serve as a primary liaison between clients, families, caregivers, and the Right at Home team, ensuring concerns and information are addressed promptly.
• Conduct initial client assessments to understand needs, preferences, goals, and home environment.
• Build strong client relationships through regular communication, follow-up, and responsiveness.
• Conduct new-hire orientations and orient caregivers to expectations, culture, policies, and quality care.
• Review reports, identify trends, concerns, and follow-up items for leadership.
• Support the Guide Program, ensuring guidance and resources reach clients and caregivers.
• Assist with HR activities, onboarding, documentation, and employee communication as needed.
• Identify changes in client needs or scheduling issues and communicate impact on care.
• Participate in client and caregiver follow-up to resolve issues before they escalate.
• Create a culture of exceptional service with responsive and trusted support.
